Gender-Affirming Surgery

Top Surgery

Chest masculinization for transgender and non-binary individuals.

Top surgery, also known as chest masculinization, is a gender-affirming procedure designed to create a more traditionally masculine chest appearance. For many transgender men and non-binary individuals, this procedure is an important and empowering step in aligning their physical body with their gender identity.

What is Top Surgery?

Chest masculinization involves the surgical removal of breast tissue to create a flatter, firmer, and more traditionally masculine chest contour. The procedure may also include repositioning of the nipples and areolas to achieve a natural male chest appearance.

Who is Top Surgery For?

  • Transgender men (FTM)
  • Non-binary individuals seeking a more masculine or neutral chest
  • Gender non-conforming individuals
  • Individuals experiencing chest dysphoria

Surgical Techniques

The technique used depends on your chest size, skin elasticity, and desired outcome. Dr. Howley will recommend the most appropriate approach during your consultation:

Double Incision with Nipple Grafts: Most common for larger chests. Involves horizontal incisions below the pectoral muscles with nipple repositioning.

Periareolar/Keyhole: Suitable for smaller chests with good skin elasticity. Results in minimal scarring around the areola.

The Procedure

Top surgery is performed under general anesthesia at our accredited surgical facility and typically takes 2-3 hours. Dr. Howley's approach prioritizes natural-looking results that align with your personal goals and body type.

Recovery

Most patients take 1-2 weeks off from work or school. Drains may be placed temporarily to prevent fluid buildup. Compression garments are worn for several weeks. Full recovery, including return to exercise, typically takes 4-6 weeks.

Top Surgery FAQ

Do I need a referral letter for top surgery?

Requirements vary. Some patients obtain referral letters from mental health professionals, while others may not need them depending on their situation. We can discuss the requirements during your initial consultation.

Will I have sensation in my chest after surgery?

Sensation may be temporarily reduced after surgery. Over time, most patients experience some return of sensation, though this varies by individual and surgical technique used.

What about scarring?

Scarring depends on the technique used. Double incision results in horizontal scars across the chest that typically fade over time. Periareolar techniques result in less visible scarring. Dr. Howley uses techniques to minimize scar visibility.
